To the Parents/Guardians of incoming sixth grade students:
NJ State Department of Health and Department of Education immunization requirements for students entering 6th grade for the Fall of 2016 are: Diphtheria, Tetanus, and Acellular Pertussis (DTaP) and Meningococcal vaccines. The details of the new requirements for 6th graders are listed below and can be found at: http://www.nj.gov/health/cd/vpdp
DTaP- every child born on or after Jan. 1, 1997 and entering Grade 6 shall have received one dose of DTaP given no earlier than the 10th birthday.
If your child received a Td booster less than five years prior, he/she will not be required to receive the DTaP booster until five years have elapsed since the last documented DPT/DTap or Td.
Meningococcal vaccine- Every child born on or after January 1, 1997 and entering Grade 6 shall have received one dose of Meningococcal vaccine.
Your child may have already received these new immunizations. Please submit records to the office of your child's current school. The immunization record must be on your health care provider's stationery. If you have questions about these vaccines, please contact your health care provider. If you do not have a healthcare provider, you can contact the Mt. Olive Board of Health for free or reduced cost immunizations. The Health Dept. can be reached at (973) 691-0900 x7353 to schedule an appointment.
Please be aware that if requirements are not met your child will be excluded from school as per NJ State Administrative Code 16.
